Participation in provider package programs with RX-O2 Hyperbaric Clinics constitutes acknowledgment and acceptance of the following terms and conditions:
1. Nature of Treatment and Risk Disclosure
Hyperbaric Oxygen Therapy (HBOT) is a medical treatment that carries inherent risks. While every precaution is taken to ensure safety and our facilities maintain an excellent safety record with over 20 years of service in the Valley, potential risks include, but are not limited to, Middle Ear Barotrauma, confined space anxiety, pulmonary oxygen toxicity, central nervous system oxygen toxicity, and fire. By participating in this program, providers acknowledge these risks and agree to appropriately inform their patients.
2. Package Purchase and Billing
All provider packages are purchased upfront at the agreed bundled rate. Treatments are drawn from the package only as prescribed by the referring provider and utilized by their patients.
3. Referral and Prescription Responsibility
The referring provider is responsible for determining medical necessity and issuing prescriptions for HBOT. Patients treated under provider packages will not be evaluated by an RX-O2 physician unless specifically requested. Consultation and follow-up visits with an RX-O2 provider are available at an additional cost.
4. Scheduling and Care Coordination
RX-O2 Hyperbaric Clinics will coordinate all patient scheduling and treatment logistics. Our certified hyperbaric technicians administer all treatments in accordance with safety standards and regulatory compliance. Chart notes and treatment updates will be provided to the referring provider to ensure continuity of care.
5. Package Validity
All packages are valid for a period of twelve (12) months from the date of purchase. Any unused treatments after this period may be subject to expiration unless otherwise agreed upon in writing.
6. Missed Appointments and No-Show Policy
Scheduled treatments that are missed or not canceled within the appropriate notice period will be deducted from the provider’s package. While every effort is made to minimize missed appointments, reserved chamber time represents a committed clinical resource. It is recommended that referring providers establish internal policies to address patient accountability for missed treatments.
7. Refund Policy
Unused treatments within a package may be eligible for refund upon request. Refunds will be calculated by removing any applied package discount and repricing utilized treatments at the standard single-session rate. The remaining balance, minus applicable processing fees, will be returned to the purchaser.
8. Non-Transferability
Provider packages are non-transferable and may only be utilized at RX-O2 Hyperbaric Clinics. Treatments cannot be reassigned to other facilities or external providers.
9. Communication and Issue Resolution
RX-O2 Hyperbaric Clinics will promptly notify the referring provider of any medical, scheduling, or billing concerns related to their patients. Both parties agree to collaborate in good faith to resolve any issues and ensure optimal patient care.
10. Commitment to Safety and Compliance
All treatments are performed using FDA-cleared, ASME PVHO-1 compliant hyperbaric chambers, operated by trained and certified personnel. RX-O2 Hyperbaric Clinics is committed to maintaining the highest standards of safety, compliance, and patient care.
By purchasing and utilizing provider packages, the referring facility acknowledges and agrees to these terms and conditions and affirms a shared commitment to safe, effective, and coordinated patient care.
